However, there will be no credit or cash refund for remaining balance of unused portion of Lyco Bucks. The Lyco Bucks can be used toward the shipping, handling and taxes if applicable. Please follow the instructions below to insure your order is processed in a timely and complete manner.

1. Print off Order Form below and complete all sections so order can be processed.
2. Serial Number(s) of Lyco Bucks must be listed on Order Form.

4. Complete back of Lyco Bucks and attach them to Order Form.
